Health In Our Hands bases all its activities around the principles of self management and in particular the principles contained in the Stanford University (California) Chronic Disease Self Management Program (CDSMP). The programs listed in the following menu’s and on our home page are developed to assist people with chronic illnesses maintain and improve their heath outcomes. Whilst running the Pathways to Healthier Living program we identified a need for such programs so that participants of the course had somewhere they could go to maintain their progress. They have proved popular and we have added to and continue to expand the range and sessions we offer. The sessions are all conducted in a very social setting and facilitated by our trained volunteers, you are encouraged to work at your own pace and we are happy to help you adapt the programs to your individual needs. We also appreciate the need for social contact and to assist that refreshments etc are always on hand. Our centre being located in the hospital enables the allied health team to refer their clients directly to our services but we are open to anyone who has the need for support in managing their health problems, our services are free but we do ask for a gold coin donation to assist with the costs of running the centre.
